Description

The SUD Pathways Coordinator supports the development and delivery of the Substance Use Disorders (SUD) Program through the Lake of the Woods District Hospital Emergency department. This includes contributing to program design, developing policies and procedures, establishing referral pathways, creating individualized care plans, and ensuring best practice approaches to assessment, treatment, navigation, and support for individuals and families experiencing substance use and/or concurrent disorders.

Beginning in the Emergency Department, this position acts as a key connection point for patients, offering rapid response to substance use concerns, safety needs, and harm reduction. The Coordinator helps facilitate seamless transitions between service providers and supports timely access to the full spectrum of healthcare services. Aligned with the five pillars that include supportive environments, peer support, harm reduction, substance use healthcare, and care transitions and navigation, the goal of this role is to improve access, enhance care quality, and support compassionate, evidence-informed services for individuals affected by substance use within the community.

This position reports to the Mental Health and Addictions Manager, SUD Program or designate.  Collaborating with the team of multi-disciplinary professionals, the coordinator will implement the key elements of the organizational Strategic Plan and the addictions program priorities. All services provided must be consistent with the Mission, Vision, and Values of Lake of the Woods District Hospital (LWDH).

Qualifications

  • Successful completion of an undergraduate degree in health sciences such as social work, nursing or other regulated health professional (master’s degree preferred)
  • Current registration in good standing with a regulatory college related to a given health profession
  • Current certification or the willingness to obtain Naloxone, BLS and CPI certification
  • Minimum of five (5) years of progressive, relevant experience in a regulated healthcare role
  • Two (2) years of recent experience in a leadership role preferred
  • Experience navigating health care systems and partnering with hospital administration
  • Experience in working with the extensive network of community health and social services
  • Excellent interpersonal and relationship building, coaching, mentoring, and team-building skills that facilitate positive interactions with health care teams
  • Excellent organizational, prioritization, and time-management skills required to coordinate multiple activities, issues, and projects simultaneously
  • Collaborative and client service oriented with the proven ability to work effectively with a diverse group of colleagues
  • Demonstrated ability to apply assessment, planning, implementation, and evaluation skills, supported by foundational and ongoing professional knowledge, to develop and deliver individualized care plans
  • Strong knowledge and understanding of concurrent disorders and associated treatment modalities
  • Demonstrated effective interpersonal and communication skills
  • Strong critical thinking skills with the ability to solve problems independently
  • Demonstrated computer skills including experience using Microsoft Office software
  • Ability to meet the physical and sensory demands of the job
  • Reliable attendance record
